Asthma Prevention

Doctors and researchers agree that the single most important thing that parents can do to prevent their child from developing asthma in the first place is to eliminate or significantly reduce dust mite allergens or asthma triggers in the home.

Dust mite allergens are known to play a major role in it's development. Researchers have determined that other important allergens (after dust mites) that also need to be eliminated from the home are cats, certain types of mold, dogs, cockroaches and tobacco smoke. It is a popular misconception that children will "outgrow" asthma. It is a severe and potentially life-threatening illness that should be taken seriously. Long term use of steroid drugs is not recommended by doctors and has serious side effects. Making these preventative environmental changes is a much more viable, healthy and preferable alternative. If you or your child are already suffering from asthma or allergies these measures will certainly help.

What are Dust Mites?
Dust mites are microscopic insects and live in bedding, pillows, mattresses, blankets, upholstery, carpets etc. They thrive particularly in humid climates. They live off of dead skin cells and produce an allergen from their excrement, which is inhaled.

All Homes Contain Dust Mites No Matter How Clean They Are.
Studies have shown that dust mite allergens in the home must be reduced to at least 2mcg/g dust or mite populations decreased to 100/g dust to have any significant benefit. There are several things that can be done to accomplish this.

What Can Be Done To Eliminate or Reduce Dust Mites?
Researchers have found through numerous studies that these are the most effective measures:
Changing the relative humidity and temperature of the home:
-Keeping relative humidity less than 50% is proven to greatly reduce the dust mite population. (Portable dehumidifiers were not shown to significantly reduce humidity).
-Mechanical ventilation with a heat recovery unit has proved to be the most effective to reduce humidity in the home.
-Using air cleaners that have HEPA (High Efficiency Particulate Air) filters, which will also eliminate other allergens such as pollen and animal dander.
-Cleaning using a vacuum with a HEPA filter and steam cleaning rugs and upholstery regularly.
-Washing all bedding in hot water (60 degrees C, 130 degrees F) on a weekly basis is known to kill dust mites. Drying bedding in a hot dryer.
-Dry cleaning quilts and blankets weekly will also kill dust mites.
-Placing plastic casings specifically made for dust mite control on pillows and mattresses.
-Chemicals used on carpets and upholstery which are known to kill dust mites include benzyl benzoate, permethrin, pirimiphos methyl, phenyl salicylate, tannic acid, common household disinfectants, combinations of these, and insect growth regulators. (please note: some chemicals can also trigger asthma and allergy reactions, therefore research them carefully before using).

Asthma is a disease that consists of two main components. These components are constriction and inflammation, both of which occur in the lung passageways. Constriction is when the airways narrow because muscles tighten around them and inflammation is when the airways get irritated and start to swell. When both of these actions occur together, they cause the symptoms of asthma such as difficulty breathing and swelling of the lungs. A person experiencing an asthmatic attack has difficulty catching his or her breath and is usually coughing or wheezing in order to try to breathe.

Asthmatic attacks can occur at any time. Some can be severe and some can simply seem like a shortness of breath. Due to this characteristic of the disease, some people are still unaware that they have asthma and need to treat it. If asthma goes untreated, a person may experience a decrease in lung function. The disease makes a person's lungs work harder and without help or treatment from an outside source, a person suffering from asthma will have weaker lungs. Air Purifiers can help.

There are many contaminants in the air that can be asthma triggers. Some of the contaminants include pet dander, allergens, pollen, dust or chemicals. When these substances are breathed in, asthmatics are sometimes affected and can experience difficulty breathing. An asthmatic affected by seasonal allergies may also find that response to the allergies can lead to an asthmatic attack. The toxins found in indoor air can easily be filtered out of the air indoors with an air purifier.

Chemical fumes, tobacco smoke and dust can be absorbed into the filter of an air purifier. With these contaminants no longer present in the air, the probability of an asthma attack is very low for the sufferer.

The High Efficiency Particulate Air Filters, or HEPA filters, that are used in air purifiers are effective in absorbing most toxins in indoor air that can be related to asthma attacks. They are able to remove 99.7% of all particles that are 0.03 microns and larger. These particles can include pollen and dust particles, which are the main cause of asthma attacks among sufferers. If indoor air pollutants are not removed from the air, they can circulate an indoor room with no escape. The pollutants are then breathed in and can cause a severe or minor asthmatic attack. With an air purifier, these particles and pollutants are absorbed and cannot be circulated throughout the air.

What is a 'HEPA' filter?

A 'HEPA' filter is a kind of mechanical filter that means it's a " high-efficiency particulate air" filter. HEPA was invented during World War II to prevent the escape of radioactive particles from laboratories. To qualify as a true HEPA filter, it must be able to capture at least 99.97% percent of all particles 0.3 microns in diameter, or larger, that enter it.

In a 1990 review of indoor air pollutants, published by The American Thoracic Society, it was stated that air purifiers containing the HEPA filter are able to remove a significant amount of particles from indoor air, making the air cleaner and safer to breathe. Asthmatics who use an air purifier in their home are less likely to have an asthma attack since the pollutants that trigger the attacks are no longer present in the indoor air they are breathing.

There is no known cure for asthma and people diagnosed with the disease will have to deal with it for the rest of their lives. However, with the help of air purifiers, people are finding their asthma attacks are becoming a scarce occurrence instead of a daily struggle. By eliminating particles in the air that trigger attacks, air purifiers are helping people with asthma to cope with their daily lives. Asthma may be an uncomfortable disease but people are finding relief in the use of air purifiers.

Ozone generators [not recommended - these types of "filters" are not reliable since their ozone levels usually exceed acceptable levels]. Although ozone technically clears the air of some particles, most groups do not recommend these. (Note: these are not ion-type filters; see "Electronic Filters" above.) These devices all exceed the acceptable level for ozone.

When you experience an asthma attack the walls of the lungs become inflamed and the mucus membranes fill with fluid and thick, sticky mucus making it difficult to breathe. Common asthma symptoms are a scratchy throat, coughing, shortness of breath, wheezing and a tight feeling in the chest. An asthma attack can be mild, moderate or severe and lasting for a few minutes, hours, or even several days.

Knowing when to get emergency help for a severe attack can save your life. If you are experiencing the following, you should seek immediate medical attention:

-medication does not seem to be helping you
-extreme difficulty breathing, talking and walking,
-your fingernails or lips are turning blue,
-chest feels tight and your ribs are pulled inward as you breathe,

Asthma triggered by cold can be helped by Ujjayi pranayama. Allergic trigger patterns can be avoided by Sitt Kari or Shitali pranayama. Kapalbhatti pranayama is very good to reduce the mucus congestion.

These breathing exercises are very good to strengthen and relax the muscles of lungs. During the asthma attack this proves to be very useful. It reduces the nerve activity in the airways causing fewer bottlenecks during the attack. It helps cleanse the air passage to breathe easily. It helps in stabilizing the autonomous nervous systems.

It is a fact that in the world we live in more and more children are suffering from asthma. Every week people die of Asthma. Often we do not know how to prevent it, and what signs to look for if we are contracting asthma.

Most common causes are thought to be dust, smoke or pollen. Even perfumes, chemicals, animal hair, and in some cases stress and nerves. Years ago many experts believed exercise help aggravate and cause asthma, although in more modern times and after more research, it is recommended on a daily basis, to help keep the body in a fit and healthy condition. If exercising with asthma, you should consult with your doctor what levels of exercise you can cope with and what you should do in order to exercise without having an attack. Often correct warm ups and warm downs should be sufficient.

Asthma is quite often frightening to either the sufferer or whoever is watching an asthmatic attack. During normal breathing, the muscles that surround the airways are relaxed, and air moves freely, where as asthmatics experience the muscles tightening around these airways, causing air not to move as freely. The slightest upset, stress, shock or anything can trigger an asthmatic attack due to asthmatics having very sensitive airways.

If you see anyone having an attack or experience one yourself you should sit leaning forward, and if possible place something warm on your chest to help relax your chest muscles. Take your inhaler or have a plastic bag to ease your breathing. If you are asthmatic you should definitely not be smoking. Reducing asthma can be helped by having ginkgo biloba, mullein, licorice root, marshmallow root or slippery elm in your weekly diet.

History

Many people that have Asthma, may have an individual and/or family history of allergies. To diagnose asthma, a doctor relies solely on taking a medical history and combines it with a physical examination.

Allergies.

Allergy testing may be may be helpful in identifying allergens, but in sensitive individuals, asthma symptoms can be triggered by inhaled allergens (allergy triggers), such as pet hair, dust mites, moulds or pollens. Patients with delayed pattern food allergy have the most persistent inflammatory form of chronic asthma. Those who have a nasal allergy often have asthma that may be triggered by it.

Developing Asthma.

Half of the people who have asthma develop it before they are 10 years old, and most develop it before 30. There are people who develop allergic asthma in their 70s and there are young children whose asthma is not allergic. Children with asthma often complain of an itchy upper chest or develop a dry cough, which may be the only sign of asthma.

Asthma Attacks.

During an attack, the smooth muscles of the lungs go into spasm, with the surrounding tissue enflamed and secreting mucus into the airways. The causes of an attack are, however, well understood by both doctors and patients. Child care providers need to recognise when a child is suffering an attack in order to prevent severe and prolonged effects. Nonetheless, an acute attack usually occurs during a cold or cough caused by a viral infection. Pay attention to worsening asthma as it may prevent a serious attack.

Smoking and Asthma.

Cigarette smoke is a powerful trigger of symptoms. Children born prematurely or those exposed to tobacco smoke are more likely to develop the disease. Prenatal and post postnatal exposure to second-hand smoke is indicated in the development of asthma. Non-smoking messages should be advertised in schools to reduce the likelihood of children smoking and thus be prone to asthma.

Stress and Asthma.

Stress could aggravate the disease in children according to a London University's research. It is well known that fighting stress helps fight asthma. People think that there is a connection between genetic makeup and the disease. There isn't - it only makes one more susceptible. If asthma is triggered by emotional stress, then a stress reduction programme could be considered. Sport during the summer causes less stress than during the winter. Exercise induced asthma occurs most often during intense activity in cold dry air.

Can it be Contolled?

Yes, it can! With good control almost all people can lead normal, active lives. Control medication on a regular basis helps prevent symptoms from occurring. When asthma is not under control, the airways become restricted and inflamed, with increased mucus production that further obstructs the airways. To keep it under control, it is important to be well educated about your asthma and how to manage it.

Known Allergens.

Allergies are triggers for asthma. If you are allergic to pets, certain foods, dust mites etc, remove them from your environment wherever possible. Be asthma aware. Every asthmatic responds differently to triggers. As a parent, try not to be overly protective of an asthmatic child. The key to living with the disease is to be very aware. If you need to carry medication make sure you always have it with you, and NEVER run our of it. Make sure you always have a backup, in the car, at home or in the office. Encourage children to emulate what you preach.

Finally, approximately one quarter of asthmatic children have their symptoms disappear as they grow older - yes it is possible to grow out of the disease!

Minor adjustments to your everyday routine can yield enormous results when it comes to preventing asthma attacks. How you ask. Let's look at just one kind of asthma. Extrinsic, or allergic, asthma is instigated by environmental triggers. Most childhood asthma falls in the extrinsic category. It is more frequent in boys than girls. Asthma is the most common childhood illness, accounting for 10 million lost school days annually. Asthma affects 5 million children in the U.S. Common culprits are
- air conditioning
- air pollution, including atmospheric ozone
- animal saliva, urine and dander
- bacteria
- chemicals (particularly hexachlorophene, formalin, ethylene diamine)
- cockroaches
- cold air
- colds and upper respiratory tract infections
- drugs (especially aspirin, cimetidine, antibiotics, beta blockers)
- dust mites and their droppings - exertion (especially if strenuous and in cold air)
- food additives or preservatives (especially tartrazine or sulfur dioxide)
- fungal spores
- fumes (e.g., paint)
As you can see, many of these causes are found in the home. So how can we do a better job fighting asthma in our indoor environments?

Here are some tips:

- Dust frequently.
- Vacuum frequently; use a cylinder vacuum instead of an upright model. Using a HEPA, or high efficiency particulate air, vacuum is optimum. These vacuums are equipped with high-spec pre-filter and filter systems; they are sealed, allergen-free and designed to remove and trap allergens such as bacteria and mold-filled dust that normal vacuums can't.
- Clean carpeting and drapes regularly. Carpet is a huge hideout for asthma allergens, harboring dust mites and pet dander--the two most common triggers of childhood asthma.
- Replace carpeting with non-fibrous flooring, such as tile or hardwood, particularly in the bedroom.
- Replace feather pillows and down comforters with those containing synthetic fibers; use a plastic mattress cover.
- Wash bedding in hot water (130 degrees Fahrenheit) to kill dust mites and remove allergens. Alternately, you can place bedding in an airtight container for 48 hours.
- Place stuffed toys in the freezer for 24 hours or in an airtight container for 48 hours to kill dust mites.
- Wear a scarf over your nose and mouth in cold weather.
- Monitor the pollen count in spring, summer and fall; avoid open fields when pollen counts are high.
- Always keep your medication with you in case of an attack.
Using an air filter can also help prevent asthma attacks. Following are some different options:
- Use an HEPA air filter. They feature a minimum particle removal efficiency of 99.97% for all particles 0.3 microns in diameter and larger. This size covers most pollen, mold spores, animal hair and dander, dust mites, bacteria, smoke particles and dust.
- Use an air purifier. They are classified according to the technology they utilize to remove different-sized particles from the air. Certain models use a chemical process, such as ozonization, to purify the air.
- Install a large room unit air purifier. These air purifiers utilize strong filters or collecting plates; some large units use electrostatic precipitation. These purifiers are highly effective at removing pollutants, particularly smoke and dirt, from large areas. They are more effective as single room units than as fixed central air filters.
- Install air filters. The efficiency of different models is measured by the size of particles they can remove from the air. They are classified in two categories: central filtration, or induct, systems and portable units with fan attachments. Central filtration systems are installed in heating, ventilation and air conditioning systems. Asthmatics should choose a HEPA filter that can trap very small and very large particles.
- Use a tabletop air purifier. These models are equipped with small panels of dry, loosely packed, low-density fiber filters and a high-velocity fan. Although these models are fairly inexpensive, be sure that the specifications match your requirements to remove very small particles from the air that can trigger asthma attacks.

Symptoms can also diverge in how rigorous they are. For instance, occasionally symptoms can be mildly annoying and at other times they can be serious enough to make you discontinue what you are doing, and sometimes symptoms can be so grave that they are life menacing and you need to get medical attention. Symptoms also differ in how frequently they occur.

Some individuals with asthma just have symptoms once every few months, others have symptoms every week, and still other people have symptoms every day. With appropriate treatment and control, still, most people with asthma can foresee to have minimum or no symptoms, and can conduct dynamic, regular lives.

Asthma Treatments - Relievers and Preventers

Asthma is a chronic lung condition that is characterized by difficulty in breathing. People with asthma have extra sensitive or hyper-responsive airways. During an asthma attack, the airways become irritated and react by narrowing and constructing, causing increased resistance to airflow, and obstructing the flow of the air passages to and from the lungs.

There are two main types of medicines for Asthma Treatments:

1. Quick Relief Medicines: also called relievers, give rapid, short-term treatment and are taken when you have worsening asthma symptoms that can lead to asthma episodes or attacks. You will feel the effects of these medicines within minutes.

2. Long-Term Control Medicines: also called preventers, are taken every day, usually over long periods of time, to control chronic symptoms and to prevent asthma episodes or attacks. You will feel the full effects of these medicines after taking them for a few weeks. People with persistent asthma need long-term control medicines.

Drugs, such as those resembling two of our hormones, can help treat asthma. These two hormones are adrenaline (epinephrine in the USA) and hydrocortisone (a steroid).

There are also other drugs which help treat asthma. Whilst drugs can remove all your symptoms if you have mild asthma, people with more severe or long-standing asthma don't get nearly such good results, so alternate medications are required.

Adrenaline (epinephrine) is pumped into our bloodstream when we have a sudden fright or emergency. It is the quick-acting hormone from the middle of the adrenal glands near our kidneys. It makes your pulse race, makes your heart thump, and readies your body for emergency action. In asthma, the medicines which resemble adrenaline quickly relieve asthma for a short time, and are called relievers.

Hydrocortisone comes from the outer part of our adrenal glands, called the 'cortex'. It is also partly an "emergency hormone" but it works much more slowly, for much longer, and in a completely different way to adrenaline. Medicines which resemble hydrocortisone slowly allow the lining of air tubes in an asthma sufferer to become normal. As a result, your asthma becomes less severe and you are less likely to get asthma attacks. So these steroid medicines are called preventers. There are other asthma 'preventers', but the steroids are the most powerful.

Most people who have asthma should have both of these kinds of medicine - preventers and relievers.

Usually the best way to take these medicines is to breathe them in. That is, you inhale them, through your nose or mouth. The reasons you inhale them are :

* because you need less of the medicine, * you won't suffer as many side effects, and, * the medicine works more quickly,

The final point is particularly important with the adrenaline-like, fast-acting relievers.

Another advantage is that the hydrocortisone-like steroid preventers you breathe in can be chosen to be biodegradable inside the body. As a result, then can do their work in the lung, but don't get much of a chance to produce any side effects in the rest of your body, because your liver breaks them down.

Quick relief medicines are used only when needed. A type of quick relief medicine is a short-acting inhaled bronchodilator. Bronchodilators work by relaxing the muscles that have tightened around the airways. They help open up airways quickly and ease breathing.

They are sometimes called "rescue" or "relief" medicines because they can stop an asthma attack very quickly. These medicines act quickly but their effects only last for a short period of time. You should take quick relief medicines when you first begin to feel asthma symptoms like coughing, wheezing, chest tightness, or shortness of breath.

Anyone who has asthma should always have one of these inhalers in case of an attack. For severe attacks, your doctor may use steroids to treat the inflammation.

Long-term control medication include:

* The most effective, long-term control medication for asthma is an inhaled corticosteroid because this medicine reduces the swelling of airways that makes asthma attacks more likely. Inhaled corticosteroids (or steroids for short) are the preferred treatment for controlling mild, moderate, and severe persistent asthma. They are safe when taken as directed by your doctor. Inhaled medicines go directly into your lungs where they are needed. There are many kinds of inhalers that require different techniques, and it is important to know how to use your inhaler correctly. In some cases, steroid tablets or liquid are used for short times to bring asthma under control. The tablet or liquid form may also be used to control severe asthma.

* Long-acting beta-agonists are another kind of long-term control medication. They are bronchodilators, not anti-inflammatory drugs. These medicines are used to help control moderate and severe asthma and to prevent night-time symptoms. Long-acting beta-agonists are taken together with inhaled corticosteroid medicine.

* Leukotriene modifiers (such as montelukast, zafirlukast, and zileuton) are long-term control medicines used either alone to treat mild persistent asthma or together with inhaled corticosteroids to treat moderate persistent asthma or severe persistent asthma.

* Cromolyn and nedocromil are also long-term control medicines used to treat mild persistent asthma.

According to research at Washington University in St. Louis, a two-drug treatment may some day help with restoring healthy breathing in people ill with asthma and chronic bronchitis.

Dr Michael Holtzman and other researchers discovered that some lining cells from the lungs air passages are able to change into another cell type, which leads to the overproduction of mucus in the airways. It was observed in mice and patients suffering from those disorders.

The researchers think that further testing of the treatments would take not less than a few years, but they claim that the combination of two drugs finally is able to preclude the pernicious transformation of these cells. It would allow the airways to function properly.

Holtzman declared the discoveries are "pushing the rock a little further up the hill" when it comes to apprehension of airway diseases.

According to Holtzman, in some people an overabundance of goblet cells is noticed because of viral infections or other factors. It leads to particular breathing disorders. He said the scientists proved that the using a combination of two types of inhibitors can block the excess cells.

One slows down the activity of a epidermal growth factor receptor - a type of a protein which is too active on the airway cells with cilia in mice with the asthma-like condition. If the protein was obstructed, the inhibitor precluded the increase of those cells.

At first reading, there isn't much extremely shocking in this report. But with a little research, in my opinion it becomes alarming.

This information comes from the Asthma Society of Canada:

"Doctors define asthma as a 'chronic inflammatory disease of the airways' . It is one of the most prevalent chronic conditions affecting Canadians. Asthma can be diagnosed at any age, but often starts in childhood. Its prevalence in Canada has been increasing over the last 20 years and it is estimated that currently over 3 million Canadians have asthma.

"The three strongest risk factors for developing asthma are family history, exposure in infancy to high levels of antigens such as house dust mites, and exposure to tobacco smoke and/or chemical irritants.

"We're all pretty familiar with allergic triggers of asthmatic symptoms, such as mould, animal dander, pollen, dust mites, etc., but perhaps less familiar with non-allergic triggers such as certain drugs, chemicals, fumes and odours, respiratory viral infections, certain weather conditions, strenuous physical exercise, tobacco smoke, and air pollution.

"Urbanization appears to be correlated with an increase in asthma. The nature of the risk is unclear because studies have not taken into account indoor allergens although these have been identified as significant risk factors.

"Experts are struggling to understand why prevalence rates world-wide are, on average, rising by 50% every decade."

Alarming? Yes. Why?

First of all, the report was updated in April of 2005, yet most of the footnoted references are from the mid to late 90's. So the report is about "Canadians" in general; children are still not identified as a unique segment of the population, their needs, habits and susceptibilities are not taken into special consideration, even though the studies all concur that asthma often starts in childhood.

Next, approximately 3 million Canadians have asthma. You have to put that into the context of how many Canadians there are, which in the year 2000 was about 31 million, with an annual growth rate of approximately 1%. So about 9.6% of all Canadians have asthma. Almost one out of 10. If one out of 10 Canadians were in a wheelchair, or wearing a cast, we would take notice. Alarming, since we treat it so casually. It's a chronic disease, which means lifelong constant management and medication.

The world's current (overall as well as natural) growth rate is about 1.3% per year, while prevalence rates of asthma world-wide are, on average, rising by 50% every decade. So asthma is occuring over 3 times more than can be attributed to population growth. These are rough figures, just to give us an idea of what we're looking at.

And this is only about asthma. It isn't about illness from chemical poisoning, or deaths from poisoning, or lung cancer, or...

So it's time to take steps to prevent asthma and other chronic environmental illnesses and diseases. At the very least, search for and buy the least toxic cleansers for household use. Get educated! Then, store all chemicals safely and securely away from children and pets.

Don't allow smoking in your home, and be aware of and avoid air pollution, both inside and out. Air out your home completely three times a day. Did you know that cigarette smoke contains about 4,000 chemical agents, including over 60 carcinogens? The U.S. Environmental Protection Agency (EPA), the National Institute of Environmental Health Science?s National Toxicology Program, and the World Health Organization?s International Agency for Research on Cancer (IARC) have all classified secondhand smoke as a known human carcinogen?a category reserved for agents for which there is sufficient scientific evidence that they cause cancer.

The U.S. EPA has estimated that exposure to secondhand smoke causes about 3,000 lung cancer deaths among nonsmokers and is responsible for up to 300,000 cases of lower respiratory tract infections in children up to 18 months of age in the United States each year. If you have to smoke, just dig your own grave, don't drag others with you!

So, become aware and clean up your home environment.
